Department Overview:
PATIENT SERVICES PHY THERAPY Home Care offers the physical therapy professional, autonomy and the opportunity to manage a patient case load with increased patient contact while providing direct care. The Rehab department has an engaged and proactive leadership team, superior technology and innovative approaches to home care. Home Care has a collaberative team approach and a supportive friendly atmosphere focused on quality client outcomes.

Work Hours:
Work Hours/Schedule (For Example: 7:00am-3:30pm or Every other weekend)

Primary Function & Major Responsibilities Supervise the total care management of clients, utilizing the comprehensive base of physical therapy. Performs medically prescribed Physical Therapy procedures, evaluations and diagnostic tests on clients. Also, provides advisory service to other professional workers who share the responsibility for that client's care. Maintains confidentiality in all aspects of the job. Maintains accurate client records. Instructs family, clients and/or other home health care professionals in appropriate home care treatment. Interacts appropriately with management and professional peers to ensure compliance with appropriate agency standards. Provides advisory and consultation services to staff to ensure quality care. Promotes personal safety and a safe environment for clients and co-workers. Performs other Duties/Responsibilities as directed.

Qualifications
Education:
B.S. degree from an accredited/approved school of Physical Therapy. Registered P.T. in the state of Minnesota by the Minnesota State Board of Physical Therapy.

Experience:
One to two years of Physical Therapy experience preferred.

Special Knowledge, Skills, and Abilities:
Strong understanding of Physical Therapy principles and practices Effective written and oral communication skills. Strong time management skills. Strong interpersonal skills. Reliable transportation to and from patient homes. Ability to lift and transfer clients using appropriate equipment. Knowledge regarding Medicare/Medicaid and third-party payors. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all age groups (pediatric through geriatric). Must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patient's needs in relation to age and to provide the care needed as described in the department's policies and procedures.
