Clinics - RI (Physicians) - HealthEast Clinics are the East Metro's premier primary care network. The medical staff includes more than 170 physicians as well as numerous certified nurse midwives. Our adult, pediatric and geriatric nurse practitioners round out a compassionate and experienced team of nurses and other support staff. Our clinics span the St. Paul area with 14 different sites and focus on providing prompt, personalized patient care with same day access for acute services. Our network size and diversity allows us to offer a full range of services for patients of all ages, including family practice, pediatrics and internal medicine.

Department Overview:
MEDICAL CARE FOR SENIORS Through HealthEast Medical Care for Seniors, a convenient specialty clinic, physicians and geriatric nurse practitioners make "house calls" to care centers, assisted living facilities and transitional care units. By caring for patients and residents where they live, our services eliminate the need to travel to a primary care clinic. Our providers include physicians and geriatric nurse practitioners who are trained to meet the unique physical and social needs of seniors.

Primary Function & Major Responsibilities Care for patients in long term care facilities. Accurately completes dictated notes and charge tickets on a daily basis. Tracks visits monthly and provides physicians with list of patients requiring MD visit. Shares after hours call with NP team. Responsible for primary health care of patients and promotion of wellness. Partners with physicians in overall patient care management to: develop protocols and guidelines to be used in patient care management; collect and document data; conduct diagnostic and therapeutic procedures; orders and schedules laboratory studies and professional consultations; and provides direct patient care services. Develops and implements a patient education program for specified diagnostic areas and incorporate clinic hours for management and education. Provides diagnosis, treatment, consultation, and follow-up for patients under the supervision of a physician. Maintains legible, accurate, confidential medical records utilizing electronic health record and/or paper as required. Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications. Maintains confidentiality in all aspects of the job. Performs other related duties as requested.

Qualifications
Education:
Valid Minnesota license as a Registered Nurse. Master's degree in Nursing. Certification from a national accredited professional nursing organization as a Nurse Practitioner with a specialty in Family Medicine, Internal Medicine or Geriatrics. Ability to meet the requirements to prescribe medications according to the Minnesota Nursing Act.

Experience:
Five to eight years experience as a Registered Nurse with two to three years of experience as a Nurse Practitioner in a clinic setting preferred.

Advanced nursing knowledge and skills, either specialized or generalized, reflected in advanced physical and psychosocial assessment, treatment, and health management; Commitment to health promotion, patient education, and collaborative approach to health care services; Demonstrated ability to work with a multi-disciplinary team and work well under pressure using diplomacy and tact; Ability to work well under pressure in a changing and challenging environment; Excellent written and verbal communication and presentation skills; Ability to work independently; Effective liaison with other health care professionals; Effective time management and problem solving skills; Knowledge of health care management and environment; Ability to utilize selected patient care software; Working knowledge of the MN Nurse Practice Act; Demonstrates knowledge and skills necessary to work with a culturally diverse group of patients and families; Ability to stand, walk, or sit for an extended period of time; Reaching by extending hand(s) or arm(s) in any direction; Finger dexterity required to manipulate objects with fingers rather than whole hand(s) or arm(s); Communication skills using the spoken word; Ability to see within normal parameters; Ability to hear within normal range; Ability to stoop, bend, push or lift 50 pounds.
