Primary Function Provides clinical coordination, case management, scheduling, and resource management to ensure that the Department of Correction patients receive appropriate efficient medical care within HealthEast. Manages the day to day operations of the Clinic and Inpatient Case Management within HealthEast for the patient to ensure the needs and expectations of HealthEast, HealthEast Providers, Correctional Medical Services, the Department of Corrections and other Clients are met. Utilizes Nursing Process to assure delivery of quality patient care to patient. Promotes and delivers high quality and well-coordinated, integrated patient care for the patient. Perform procedures and treatments as directed by the provider according to established clinic protocols and OSHA standards. Completes and maintains patient medical records. Promotes personal and patient safety by using knowledge of hospital and unit infection control policies, safety policies, and the practice of universal precautions. Provides coordination of inpatient care in collaboration with HealthEast Services (i.e.: social service, utilization review), Correctional Medical Services, the Department of Corrections Health Service Departments and other Correctional Clients to assist in the discharge planning process to ensure the correctional patient is transferred to the appropriate level of care within the corrections healthcare system. Actively participates in assertive utilization management through a partnership with social services, clinical managers, physicians, nursing staff and other clinicians. Communicates on an ongoing basis with clinical staff, administration and HealthEast Care Inc., to ensure effective relay of information pertinent to the care of the patient. Maintains confidentiality in all aspects of the job. Performs other related duties as requested to meet department needs.

Qualifications
Education:
Graduate of an accredited school of nursing. Current license to practice as a Registered Nurse in Minnesota. Bachelor's Degree in Nursing or a related healthcare field.

Experience:
Five years of recent Acute care experience. Two years of previous management experience. Previous quality management/utilization management experience. Special Knowledge,

Skills and Abilities:
Knowledge of common nursing procedures and use of equipment; Demonstrated ability to work in a clinical setting for a variety of specialties; Ability to apply knowledge and nursing judgment in providing nursing care based on the nursing process; A working knowledge of interdepartmental and hospital relationships; Ability to work effectively with physicians, clinical staff, clients and patients; Knowledge of continuous quality improvement, utilization and resource management; Ability to problem-solve effectively and to assist others in developing problem-solving skills; Strong communication and interpersonal skills. Ability to demonstrate effective written, presentation and computer skills; Basic Life Support.
