Clinics - RI (Physicians) - HealthEast Clinics are the East Metro's premier primary care network. The medical staff includes more than 80 physicians as well as numerous certified nurse midwives. Our adult, pediatric and geriatric Nurse Practitioner (CNP/NP)s round out a compassionate and experienced team of nurses and other support staff. Our clinics span the St. Paul area with 11 different sites and focus on providing prompt, personalized patient care with same day access for Acute services. Our network size and diversity allows us to offer a full range of services for patients of all ages, including family practice, pediatrics and Internshipal medicine.

Primary Function Provide Primary Care and Consultative services to patients who require hospitalization and their Primary Care MD does not round at St. Johns. Works closely with specialists to clinically co-manage hospitalized patients. Demonstrates exemplary communication skills with community primary care physicians to ensure continuity of care post discharge. Works closely with interdisciplinary teams on all units. Maintains confidentiality in all aspects of the job. Ensures referred and assigned patients are properly and efficiently admitted and receive appropriate and responsive medical care according to their condition, symptoms, and diagnosis. Completes effective and timely verbal communication with the patient's primary physician at various stages during the process, such as admission, care conferences with families, change in patient condition and upon discharge. Functions as a member of the Clinical Care Team to achieve patient outcomes within established parameters, through the development, communication and the implementation of the plan of care. Document patient diagnosis, treatment, orders and clinical progress properly and correctly in order to allow for proper and correct billing. Submits charges daily and responds to coding feedback timely. Functions as a spoke person for Hospitalist program and for St. John's hospital in a variety of settings, both internal and external, to improve awareness of program and benefits to patient, physicians, the community and the hospital. Provides services to patients in other settings as required to ensure quality and the continuum of patient care while ensuring patient safety and satisfaction. Works closely with the Medicine Service Care Coordinator to ensure care progression and that the Hospitalist Service meets and exceeds the needs of its customers. Performs other related duties as assigned.

Qualifications
Education:
Physician with MD or DO degree; Board eligible or board certified in Internal Medicine or Family Practice. MN license and DEA required.

Experience:
Hospital based inpatient management experience.

Special Knowledge, Skills, and Abilities:
Excellent written and verbal communication skills, ability to work independently, effective time management and problem solving skills, knowledge of health care management and the hospital care environment, experience working in a total quality improvement environment, demonstrated leadership skills. ACLS certification.
