PATIENT SVCS-SKILLED NURSES Home Care offers the nurse professional autonomy and the opportunity to manage a patient case load with increased patient contact while providing direct care. Work Hours/Schedule (For Example: 7:00am-3:30pm or Every other weekend)
Primary Function & Major Responsibilities Supervise the total case management of palliative care patients, utilizing comprehensive base of nursing theory and nursing process. The nursing process includes: observation, assessment, planning, implementation and evaluation, as evidenced through documentation. Reviews patient referral information and responds to inquiries as appropriate. Performs patient assessment based on HealthEast Home Care's conceptual framework of practice to ensure effective and appropriate home care services. Directs, plans, and initiates appropriate action independently and responsively in home care situations. Manage/supervises a team of RNs, LPNs, and HHAs to provide effective and appropriate home care.
Qualifications
Education:
Current unrestricted RN licensure in State of Minnesota in PHN, BSN, BA in nursing preferred.
Experience:
One year nursing experience in acute/skilled facility within the past five years required, two years preferred. Additional home care and supervisory experience preferred. Regular/Temporary
